A note for every visit, checked before it counts

Visit notes and service logs for APD caregivers

Caregivers write the visit note on their phone while they're clocked in: by talking, by tapping through a few questions, or by typing. A finished visit then waits for your office to approve its note, and you can check it against what a Qlarant reviewer looks for first.

Visit note

Eleanor chose to make a grocery list before leaving home. At Fresh Market she found four of six items on her own and asked a clerk where the rice was, with a reminder from me. She said, “Next time I’ll bring my own bag.” She ate lunch at home and had no incidents.

Meets every item of the checklist.

How it works

From the caregiver's phone to an approved visit

The note gets written where the visit happens, and the office sees it the same day instead of at the end of the month.

Writing the note

Three ways to write, none of them a blank page

After clocking in, the caregiver opens the note and picks how to write it. Voice note: they talk, and the AI fixes the wording and order without adding anything. Guided note: one quick question per screen, then the AI writes the note from their answers. Prompt: tell the AI what to write, or what to change in the note so far, with undo.
  • Notes open only once the caregiver has clocked in
  • A saved note is locked; the caregiver can't change it later
  • On the iPhone, unfinished notes are kept on the device, encrypted

Guided notes

Questions about this person, not a generic form

Each iBudget service has a ready-made note template built on Qlarant's standards, and you can edit it on the web. The guided questions pull from the person's own notes guidelines: their goals in their own words, and the supports, activities, places and people the office has approved. The caregiver taps what happened and how much help was needed, so the note says it exactly.
  • Level of help per support: on their own, reminder, verbal guidance, physical help
  • The AI is shown the person's recent notes so it words this one differently
  • Every sentence is checked against the caregiver's answers; nothing is made up

Checking the note

See what's missing before you approve

Open a finished visit on the web and check its note. Waiversoft reads it for the items Qlarant scores: the exact support given, the person's response, choices offered, goal progress, health and safety, people-first language, and whether it describes this visit. It also warns when a note is very close to another one. If something is missing, Fix asks Coworker to rewrite the note using only what it already says.
  • Each item shows as met, missing or an issue, with the reason
  • Coworker shows the full fixed note before anything is saved
  • A fixed note still waits for your approval

Approving visits

Every finished visit waits for an approved note

A visit with a clock-out shows Needs approval until an admin approves its note. The Needs approval list in Scheduling starts from the oldest waiting note, so nothing sits for weeks. Each visit also has a one-page service log PDF with the times, service, units, caregiver and note. Not sure how that differs from a progress note? Read service log vs. progress note.
  • Approve as is, or edit the note and approve in one step
  • Mark incomplete puts an approved visit back in the list, until it's billed
  • Office staff can add or correct a note; an admin approves it

The details that keep notes reviewable

Spanish and Haitian Creole

Caregivers can see the note helpers in Spanish or Haitian Creole. A voice note stays in the language spoken, with one tap to translate it to English.

People-first wording

Guided notes use the person's name and respectful words, never "the consumer", "non-compliant" or a vague "assisted with".

Approve before billing

Optional: visits whose note isn't approved are left out of claims until it is.

Templates by service

A ready-made note template for each iBudget service. Change the sections and wording on the web.

Saved notes stay put

Once the caregiver saves, the note is locked. Corrections come from the office.

Approvals on the record

Approving a visit or marking it incomplete is written to the audit log.

Follow-up on the service log

Any health and safety follow-up from the visit prints on that visit's service log.

Web and iPhone

Caregivers write notes in the iPhone app or in the caregiver app in any phone's browser.

Getting started

Notes your reviewer can follow

01

Write each person's guidelines

Add their goals in their own words and the supports they use. Guided notes ask about these.

02

Caregivers write at the visit

They clock in, do the visit and write the note by voice, guided questions or typing before they leave.

03

Check and approve

Work down the Needs approval list. Check each note, fix what's missing, approve the visit.

FAQ

Questions about visit notes

Does the AI make up what happened at the visit?

No. A voice note only fixes the caregiver's wording and order. A guided note is written from the caregiver's answers, and each statement is checked against them; anything they didn't say is cut. When the office uses Fix, Coworker may only use what the note already says.

What if a caregiver isn't comfortable writing in English?

Their note helpers can be set to Spanish or Haitian Creole. Guided notes are written in English. A voice note keeps the language they spoke, and Translate to English turns it into English before they save.

What happens to a visit whose note isn't approved yet?

It shows Needs approval on the schedule and in the Needs approval list. If you turn on the approve-before-billing preference, it also stays out of your claims until an admin approves the note.

Can we change a note after it's approved?

Yes. Mark the visit incomplete and it goes back to Needs approval, so you can correct the note and approve it again. A visit that's already been billed can't be reopened.

Do caregivers need an iPhone?

No. Caregivers can use the Waiversoft iPhone app or open the caregiver app in their phone's browser, including on Android phones.

What does it cost?

Visit notes are part of the one Waiversoft plan, $20 per consumer per month. Office staff and caregivers are free.
