Nursing and therapy codes, counted the APD way

Waiversoft schedules, clocks in and bills the nursing and therapy services you provide under the iBudget waiver, alongside your agency's other services. Each authorization line knows how its code is billed: a private duty nursing shift counts quarter hours, a skilled nursing visit counts once, and a therapy evaluation counts as one assessment.

How it works

Built for codes that don't all bill the same way

Nursing and therapy codes mix quarter hours, single visits and assessments. Waiversoft keeps track of which is which, from the authorization to the claim.

Authorization lines

Every nursing and therapy code, ready to pick

When you add a consumer's authorization, pick the service from the iBudget rate table effective July 1, 2026: Private Duty Nursing (T1000UC), Residential Nursing (T1001UC, T1002UC), Skilled Nursing (T1001UCHM, T1002UCHN), Occupational (97530UC), Physical (97110UC), Speech (92507UC) and Respiratory Therapy (S5181UC), plus their RN assessments and therapy evaluations. The preset fills in the code, rate and how the line is billed; every field stays editable so the line matches the authorization you were given. For what each service covers, see our iBudget waiver services overview.
  • LPN and RN codes as separate presets, with their own rates
  • Billed per quarter hour or per visit, set on each line
  • Units left this month and on the whole authorization

Units

Quarter hours, visits and assessments, counted right

On quarter-hour lines, like private duty and residential nursing or therapy sessions, a visit's units are its clock minutes divided by 15 and rounded: 53 minutes is 4 units, 7 minutes is none. On skilled nursing visits and therapy evaluations, which the rate table bills per visit or per assessment, each visit is one unit however long it runs. The same count is used for units left, scheduling, billing and pay, so they never disagree.
  • A skilled nursing visit uses one unit of the authorization
  • An RN assessment on a quarter-hour code still counts its minutes
  • Visits too short for one billable unit are flagged, not dropped silently

Scheduling and clock-in

Nurses and therapists on one calendar

Book an eight-hour private duty shift and a weekly therapy session on the same calendar, each on its own authorization line, and see the units left before you save. Repeating shifts cover a standing schedule; conflict warnings stop you double-booking a nurse. Staff clock in and out from their phone, and each punch is compared with the consumer's approved visit area. More on scheduling and clock-in.
  • Open shifts show until a nurse or therapist is assigned
  • Staff can start an authorized visit that wasn't on the schedule
  • Correct clock times after the fact, with every change logged

Billing

Nursing, therapies and everything else in one claim file

Pick the billing period and Waiversoft builds the 837P claim file from the visits' clock times, each line billed by its own unit. Before you download, the claim preview shows every line and why any visit was left out: no note, a note waiting for approval, or no clock-out. Prefer to key claims by hand? Download the claim entry worksheet instead. See billing and claims.

Licenses with expirations

Add RN, LPN and therapist licenses as your own employee document types; they turn amber 30 days before they expire and red after.

Your own consumer documents

Add the plans and orders your services rely on as document types on the consumer record, with expiration tracking.

A note on every visit

Nursing and therapy visits use the general note template: supports given, goal progress, the person's response, health and safety. Edit it for your agency.

Approve before billing

Optional: visits aren't billed until the office approves their note.

Pay by the hour or the visit

Set an hourly or per-visit rate for each employee, with a different rate per service.

Daily hour limits

Cap a staff member's daily hours per service; going over needs your approval.

Office-entered times marked

Visits the office completed by hand are flagged in billing as office-entered, not from the phone.

FAQ

Questions about nursing and therapies

Which nursing and therapy codes can we add?

Every nursing and therapy service on the iBudget rate table effective July 1, 2026: private duty, residential and skilled nursing (LPN, RN and RN assessment codes), occupational, physical, speech and respiratory therapy, and the therapy evaluations and assessments. You can also type a code and rate by hand.

How does a per-visit skilled nursing code bill?

As one unit per visit, however long the visit runs. Quarter-hour codes on the same consumer, like private duty nursing, still count their minutes.

Is there a nursing or therapy note template?

Not a separate one. Nursing and therapy visits use Waiversoft's general visit-note template, which you can edit for your agency.

Can we track nursing and therapist licenses?

Yes. Add them as your agency's own employee document types and Waiversoft tracks each expiration date, like every other document in the file.

What does it cost?

Waiversoft is one plan, $20 per consumer per month. Office staff and caregivers are free.
