Supported Living Coaching: the APD provider's guide

Coaching adults to live in their own homes comes with the most paperwork of any in-home service: an assessment, a housing survey, a financial profile, quarterly home visits — and Qlarant checks 29 standards.

Supported Living Coaching helps adults 18 and older find housing and learn to live on their own — daily living, chores, meals, shopping, money, and social skills. It's 1:1, billed by the quarter hour (up to 6 hours a day), and needs a coach with a degree or equivalent experience and APD's supported living pre-service course. What sets it apart is the documentation: a functional community assessment, a housing survey before the lease, a financial profile, quarterly home visits with a health and safety checklist, and daily progress notes.

Who qualifies, and where they live

Adults who live in their own home or apartment, or who have a goal to move there — coaching can start up to 90 days before the move, even while the person still lives with family or in a group home. The home has to be available to anyone for lease or purchase, the person's name goes on the lease, and neither you nor your family can own it. The person may share it with no more than two other people with developmental disabilities, and the coach can't live there. A home that fails the housing survey can't be authorized. When the person also gets Personal Supports, the providers coordinate through the WSC — and Qlarant looks for proof.

Who can coach

Solo providers and agency employees, with one of: a bachelor's in education, rehabilitative science, business or a related field; an associate's (or two years of college) plus two years of developmental disabilities experience; one year of college plus three years; or a diploma plus four years of experience. Coaches complete APD's Supported Living Pre-Service course before serving anyone, 8 hours of in-service a year, and Social Security work incentives training in their first year. Providers keep 24/7 on-call coverage, a named backup provider (who bills for the time they cover), and toll-free phone access.

The documents, and when they're due

  • Functional Community AssessmentBefore the move — or within 45 days of starting service for someone already in supported living — then at least yearly
  • Housing SurveyBefore the lease is signed; the WSC approves it after an on-site inspection, and it's reviewed at each quarterly home visit
  • Financial ProfileTo the WSC within 10 days of starting service; updated when housing is chosen, when finances change, and at least yearly (regional approval of any subsidy before the lease)
  • Implementation planWithin 30 days of starting or of receiving the support plan, then yearly — plus service frequency, safety needs and a personal emergency plan, 24/7 contact, and natural supports
  • Daily progress notesEvery service date, in the record before the claim — activities, contacts, times, summary, follow-up, and goal progress
  • Quarterly home visits and Health & Safety ChecklistEvery quarter, signed by the person, the coach and the WSC
  • Quarterly summaries and annual reportEvery quarter (or monthly); the third is the annual report, due to the WSC 60 days before the support plan's effective date

Billing and rates

Quarter hours, 1:1, up to 24 a day and 8,760 a year; rounding to quarter hours happens once, at billing. The agency rate needs two or more employees — subcontractors don't count.

The 29 standards Qlarant scores

Qlarant's Supported Living Coaching tool (version 7, July 2026) is the longest of the in-home services. It checks progress notes against claims; the implementation plan's timing, contents, delivery and use (including documenting when the support plan arrived); summaries and the annual report 60 days ahead; authorizations and the daily and yearly caps; the lease and ownership rules; the functional community assessment, housing survey and financial profile; quarterly home visits and the health and safety checklist; Personal Supports coordination; the right rate and 1:1 delivery; billing after service; nine person-centered standards (health, safety, abuse and neglect, choices, community, rights); and progress notes that aren't copies of earlier ones.

Its September 2026 checklist asks for daily notes, current and previous implementation plans with proof they're followed, support plans, summaries and the annual report, assessments, the lease or mortgage, the housing survey, quarterly home visits and health and safety checklists, financial profiles, Personal Supports coordination evidence (emails, texts, notes), an annual abuse and neglect discussion — and six months of payroll if you bill the agency rate.

What is Supported Living Coaching?

Can someone get Supported Living Coaching while living with family or in a group home?

Only if they have a goal to move into their own home, and only for up to 90 days before the move — that's the one window where coaching can overlap residential habilitation or family-home living.

What are the lease rules for supported living?

The person's name must be on the lease (or mortgage or deed), neither the provider nor the provider's immediate family can be the landlord or have an ownership interest, the coach can't live in the home, and the person may live with no more than two other people with developmental disabilities. A home on the family's property needs a regional waiver, a separate structure and a separate lease.

What training does a supported living coach need?

APD's Supported Living Pre-Service course before serving anyone (one-time), 8 hours of in-service each year, and “Introduction to Social Security Work Incentives” within a year of enrollment, which counts toward in-service. Coaches also need a degree or the experience equivalent set in the iBudget Handbook.
