Residential Habilitation: the APD provider's guide

Residential Habilitation is supervision and specific training in daily-living skills for people who live in an APD-licensed foster home, group home, or residential habilitation center, delivered under a written implementation plan. It's billed by support level (Basic through Extensive 2, with higher Behavior Focused and Intensive Behavior tiers), daily or — at 24 or more days in the month — monthly. Since 2015 the provider must be the owner or operator of the licensed home; getting the license is covered in how to open an APD group home.

A person's level comes from APD's residential habilitation rate descriptors — functional, behavioral, physical and other needs — and changes need medical necessity. Each level carries a minimum number of direct-care hours per person per day, totaled across the home and met over a seven-day week. Only training, intervention or supervision time counts, and your schedules and payroll are the proof.

Behavior Focused is for residents who, in the past 12 months, showed at least one listed characteristic — self-injury or harm to others needing medical attention, arrest, property damage over $500, life-threatening behavior, restraint or emergency medication, or sexual behaviors — as determined by the APD regional behavior analyst and re-checked at least yearly. The home is APD-designated; a qualified behavior analyst monitors face to face; at least 75% of direct staff complete 20 contact hours of ABA training (RBT certification substitutes) plus an approved emergency-procedures curriculum; and half of trained staff get monthly competence checks. It's meant to be a temporary placement.

Intensive Behavior is for behavior exceptional in intensity, duration and frequency in the past six months (for example injuries needing emergency care, fire setting, suicide attempts, repeated restraint). It needs a Level 1 program director, no more than 20 residents per full-time behavior analyst, trained staff with a diploma, staffing at least equal to Behavior Focused Extensive 2, and a written transition plan — and the rate covers all adjunct behavioral, psychiatric and nursing services. Its level (1–6) is scored by the regional behavior analyst on APD's behavioral service need matrix.

• Daily attendance log, sent monthly — no times needed, but it has to support every day billed.
• Implementation plan within 30 days of starting the service or receiving the support plan, then yearly; a signed copy goes to the person (or representative) and the WSC by day 30.
• Quarterly summaries (or monthly, if you choose). The third quarterly is the annual report, due to the WSC 60 days before the support plan's effective date.
• Staffing schedules and payroll proving the direct-care hours; the licensing rule keeps weekly staff schedules for six months.
• Behavior Focused and Intensive Behavior homes add the behavior analysis eligibility form, the current behavior plan, monthly graphs, and records of staff training on the plan.

Standard and Behavior Focused rates are the same for solo and agency providers. Bill the monthly rate at 24 or more days present, after the month ends; otherwise the daily rate. Room and board is paid separately — not by Residential Habilitation. Intensive Behavior is a statewide day rate by level (Level 1 $367.21 through Level 6 $550.80). The live-in rate applies only to licensed homes with three or fewer residents.
